Gynecomastia and Its Types

Gynecomastia is a condition in which the breasts become abnormally large. It occurs due to hormonal changes or it can be hereditary. Other conditions that can lead to gynecomastia are obesity, drugs, medications and tumors. It can occur to the people of any age.

As far as the physical discomfort is concerned, Gynecomastia does not lead to any; but it can make the person less-confident and emotionally depressed.

Gynecomastia is of two types:

Physiologic gynecomastia

Physiologic gynecomastia occurs due to altered estrogen/androgen balance. It can also occur when the breast tissues get sensitive to normal estrogen levels. It is common in the newborns, adolescents and older men. Though it is self-limited, it can be treated in order to minimize physical discomfort and emotional distress. Usually, it takes one and a half year to get physiologic gynecomastia effectively resolved. However, physiologic gynecomastia can remain till adulthood in the case the tissue causes pain and is larger than 4 cm.

Pathologic Gynecomastia

Also known as non-physical gynecomastia, pathologic gynecomastia is caused due to chronic conditions (including renal insufficiency, hypogonadism, cirrhosis, etc.), supplements, medications, illicit drugs, and in some cases, due to tumors. It occurs when the estrogen increases too much or the androgen level decreases to a great level.

Gynecomastia treatment

Gynecomastia can be effectively treated with surgery. It is performed under general anaesthetic. The surgeon explains the procedure and the risks to the patient before starting the surgery. Then liposuction of the breast tissue is performed. In the case, the tissue is glandular, it is removed by making a small incision and all the breast that lies under the nipple is removed. If the breasts are fatty, liposuction is enough to get rid of the breast gland.

An ultrasonic liposuction is also an excellent approach. In this, the extra skin contracts and gets remodeled over the chest within a period of six weeks. And the tissue needs to be removed if the amount of the extra skin is large. The procedure takes 1-3 hours depending on the amount of the tissue that needs to be removed.

Applications of Fat Transfer Procedure

Fat transfer, also known as fat grafting, is a common plastic surgery procedure in which fat is taken from one part of the body, is purified and then is injected into the target area. The procedure can be used on different body parts including face, hands, hips, breast and more.

Though the procedure dates back to 1893, the improved and modern technique for the procedure was introduced in 1986. And nowadays, the fat transfer procedure has become common in UAE and other parts of the world.

Types of fat used in the fat transfer procedure

The types of fat used for fat transfer are:

  • Macrofat

It is used for the enhancements and augmentation of larger body parts, breasts and buttocks.

  • Microfat

It consists of smaller fat particles and is injected into the delicate body parts, like face and hands.

  • Nanofat

Nanofat also consists of small-sized fat particles which are injected into the delicate part of the body. This type of fat does not cause lumpiness.

Apart from the fat, different types of tissues can also be injected. The most common types of tissues that are used in the fat transfer procedure are Fascia and Dermis.

Applications of Fat Transfer procedure

In UAE, fat transfer is used for a range of treatments and is performed on different body parts. Here we have mentioned the applications of fat grafting according to the body parts.

1. Face

 - Restores youthful appearance
- Repairs scars
- Adds volume to the wrinkled areas and cheeks
- Improves the appearance and texture of the skin

 2. Hips

- Adds volume to hips to enhance
- Improves waist-hip ratio

3. Hands and Feet

- Improves appearance
- Reduces wrinkles
- Provides fat padding for bony feet

4. Buttocks

- Makes the buttocks bouncy
- Increases size

The fat transfer procedure can take anywhere between 1 to 3 hours and the patients are usually discharged after a day. However, they need to follow some precautions in order to prevent the migration of fat to another area. Patients can experience swelling and bruising, which can be controlled with ice packs.
